Long Term Conditions



There are a large number of our patients who have two or more long term conditions.


These include:

High blood pressure, Heart disease, Heart failure, Stroke/TIA, Dementia.


To help these patients we will run 'Long Term Conditions Clinics'.

Any patient with two or more of these conditions will be invited to the Long Term Conditions Clinic.

The aim of these clinics is that patients will be seen once a year for a full review.  They will have a nurse appointment when all their long term conditions and medication will be reviewed.  Any relevant investigations, ie. blood tests etc will be arranged one month prior to the appointment.  All the investigation results will be reviewed by our Pharmacist and a GP prior to your appointment.


The clinics will be run by our qualified Nurse prescriber and our LTC Nurse.  Patients will have their health and medication reviewed and will receive a personalised care plan to help them.
